High Number of Acute Respiratory Diseases Cases in Singapore in Q4 2025

In the last quarter of 2025, Singapore experienced a noticeable rise in acute respiratory infections, with more people seeking medical care for flu-like symptoms compared with previous years. This increase reflects a growing burden of respiratory illness in the community, particularly during what is typically considered the tail end of the year.

Data from the Communicable Diseases Agency (CDA) show that average daily polyclinic attendances for acute upper respiratory infections between October to December were consistently higher than the historical median for the same period from 2020 to 2024. Notably, the increase in acute upper respiratory infections in December 2025 was steeper than that observed during the same period in 2020–2024, suggesting a more intense and less predictable seasonal surge. Specifically for influenza, Channel News Asia (CNA) reported that cases began rising sharply from late August 2025 — months earlier than the usual December to March flu season. 

 

Factors Contributing to Faster Spread of Respiratory Infections

Experts note that year-end damp weather can create favourable conditions for virus survival. In addition, ongoing viral evolution may have contributed to increased transmission in 2025, as genetic changes can affect how easily respiratory viruses spread or partially evade existing population immunity. Together, these factors may cause respiratory infections to spread faster and affect more people, contributing to the heightened disease burden observed in late 2025.

As we step into 2026, the number of cases reported for acute respiratory infectious disease in the 2nd week of January is 22% higher than the historical median.
